Schedule Your Visit with MidloMed 1 of 4 Ways

After booking your appointment: You will receive a welcome email to create your portal account and a text with the link to complete the required forms for your visit. Forms can be completed on your phone/device using the link provided via text or you can complete them within the portal.

Before Your Visit: Please complete all forms in advance, including the credit card authorization and, if applicable, the insurance billing authorization.

Card on File: A valid credit card is required on file for every patient. 

Payments

Accepted Payment Methods: We accept all major credit cards, debit cards, and health spending cards (HSA, FSA, HRA).

Self-Pay Visits: You will receive a payment link via text before your appointment. Please complete payment upon receipt prior to or at the time of your visit.

Insurance Visits: Your credit card on file will not be charged before your visit. However, it may be used to cover applicable fees, such as your copay, at the time of service as well as balances due after billing your insurance, where appropriate.

We are open Monday through Saturday with extended hours/evening availability!

Traditional Office Hours: Mon, Wed, Fri  9 am - 5 pm

 Evening Availability: Tuesdays & Thursdays 11 am - 7 pm

Weekend Availability: Saturdays 11 am to 3 pm

Most visits are available in-person or virtually via phone or video

In Person Availability: 

Available all open hours/days except for Saturdays   

Telephone & Video Visits: 

Available all open hours/days including Saturdays

For Telephone Visits: 

We will call you at your appt time using the number provided.  

For Video Visits:

You can join the video visit within the patient portal by clicking the video button or by clicking on the link provided via email and/or text message. The provider will join you shortly after. Please allow up to 15 min for the provider to join as they may be running a little behind with another patient.

Please note: Your ID is required for video visits and must be shown in order to start the visit. Please make sure to have your ID on hand before your video visit.

We offer a convenient self scheduling option & same day scheduling!

Self schedule your visit same day:

You can schedule your visit through the website or patient portal with only 4 hours notice and be seen that day.

Avoid the hassle and having to wait: When you self schedule your appointment, it is confirmed immediately. You do not have to wait for a reply or to speak to a person.

Schedule any day and at any time: With self scheduling you can schedule your appt any day of the week and any time of the day, whether the office is open or not.

 Inclement Weather 

Even if the office closes due to inclement weather, you can still be seen. We are always available during regular office hours for virtual visits by  phone or video, no matter what is going on outside. During these times, self scheduling may not be available but all you need to do is give us a call.  

In person visits already scheduled will also not be cancelled. Rather patients will be given the option to change to a virtual visit or reschedule their in person appt to another day, if preferred. 

Unable to make your scheduled in person visit or running late and wont be able to make it in time?

No problem! 

Give us a call and we can switch it to a virtual visit via telephone or video, even if at the last minute. 

Cant leave wherever you are or cant make it out for other reasons?

 You wont have to and can still be seen
