** Please note, visit fees shown above cover standard services only. Additional services may result in extra charges. Charges for labs, imaging, and medications are not included unless specifically stated otherwise. If insured and plan to pay out-of-pocket for your visit, most insurance plans will still cover the cost of ordered labs, imaging, and prescriptions medication as usual—provided these services are covered and allowed under your plan.**
